Vaccination Documentation and Exemption Requests

Requests for religious or medical exemptions must include supporting documentation with the request to assist LSU Health Science Center New Orleans in determining whether a reasonable accommodation is required for a student with a medical condition or a sincerely held religious belief.

COVID Vaccination proof should be sent to Student Health. All new students submit this information in their immunization history packet.
