COVID-19 Self Reporting
For the health and well-being of all those on the LSU Health New Orleans campuses – in New Orleans, Baton Rouge and Lafayette -- we are asking all members of the Health Sciences Center community who are under suspicion of or diagnosed with COVID 19, to self-report. We will respect your privacy as we conduct confidential contact tracing to notify work- or school-related colleagues who may have been exposed.
Do you think you may be experiencing COVID-19 symptoms?
Or
Have you had a positive COVID-19 test?
Or
Are you under self-isolation or quarantine for possible or proven COVID 19?
Please call the Campus Health Clinic (504 412-1995) to discuss the need for testing, continued quarantine or medical care.
If employees or students have had a positive COVID test, they must also fill out this COVID-19 positive test self-reporting form to facilitate contact tracing. An LSUHSC health care worker will contact you. Managers also may complete this form for their employees.
COVID-19 Positive Test Self-Reporting Form
Students should fill out the student self-isolation reporting form if they are self-isolating because of COVID symptoms, a COVID positive test, or exposure to someone with COVID. This will help the Student Health Office track the spread of disease on campus and facilitate communication with Student Affairs.
